Have not been able to make "Never Show Again" work for many rev levels back...on Vista Business or XP-SP2.
I just tested it on Vista BE, and will try on XP Pro in a few minutes.

The bugs also occur with XP Pro.
It fails if I either:
1. Run the Registry Cleaner, then try the Never Show Again, and then rerun the Registry Cleaner, or
2. I close the program between the runs.
After I have run the scan and clicked Never Show Again, I do not have a RegSuperIgnoreList.dat! However I do have a SuperRegIgnoreList.dat whose contents are:
HKEY_LOCAL_MACHINE\Software\Borland\BLW32\LOCALE_LIB1
HKEY_LOCAL_MACHINE\Software\Borland\BLW32\LOCALE_LIB2
HKEY_LOCAL_MACHINE\Software\Borland\BLW32\LOCALE_LIB3
HKEY_LOCAL_MACHINE\Software\Borland\BLW32\LOCALE_LIB4
HKEY_LOCAL_MACHINE\Software\Borland\BLW32\LOCALE_LIB5
These are the indeed the correct items that I wish to always ignore. By the way, I did try renaming SuperRegIgnoreList.dat to RegSuperIgnoreList.dat, but no luck.
I am running on WinXP Professional SP2 with all updates/KBs installed.

Could you try the same thing with the Structured list format? Also, do you always try to hide (i.e. use the Never Show Again) the same items? If you do, could you try to hide something else?
You can later always make the items show again by deleting (or modifying) the SuperRegIgnoreList.dat file.
Bugs A, B, and C
If you first do the "Never show" action using the Structured list, the bugs do not occur, and, in a later PT session, the "Never show" list is processed correctly by the "Raw list."
However, additional "Never show" items chosen in that "Raw list" session are lost when you close the main window (that's Bug C).
I can confirm that, as usual, tullik has described the problems clearly and succinctly!
Additionally, as an FYI, I ran on WinXP Professional SP3 with exact same results. I do not believe you will experience any surprises with the release of XP SP3.
